Transferring from the acute care setting to a post-acute care setting can be stressful. Our transitional care coordinators ensure that the transition to The Trails is seamless. Prior to admission, the patient and their family are invited to tour our transitional care unit and meet with our nursing and therapy staff. To ensure that our patients are comfortable and satisfied at all times, our coordinator communicates with you prior to admission, upon admission, and during the course of your stay. We believe that taking steps to alleviate unnecessary anxiety results in a pleasant and speedy recovery back to home.
