We are dedicated to the holistic plan of care that will strengthen the mind as well as the body. Our experienced staff recognizes the difficult transition for our patients as well as their families. Our professional caring team is happy to share their expertise with you.
We are a 65-bed skilled nursing and rehabilitation center certified by Medicare and Medicaid. We also participate with many Health Plans and accept private pay residents. For over 23 years, we have helped hundreds of members of the community get back on their feed after an illness, injury or surgery. We have also welcomed hundreds more as permanent residents when their health condition and care needs made it impracticable for them to live at home or with a family member.

Medical Rehabilitation Services
At Prescott Nursing and Rehabilitation Community, we are dedicated to rehabilitating each patient in an environment which promotes independence and safety. Licensed nurses are on-site around the clock.
Our medical rehabilitation program provides high-acuity nursing care for post-acute, clinically complex patients. With a focus on short-term rehabilitation stays, many of our patients return home independently.

Specialized Services
Care Pathways Program for Discharge Planning
Prescott Nursing and Rehabilitation Community is a leader in care coordination. We utilize a program of Care Mapping to develop individualized care plans which specify clinical and rehabilitation goals that are measurable and drive the management of each patient’s care.
Through our Care Mapping process, the discharge disposition of the patient is discussed with the inter-disciplinary team within the first 24 hours of admission. At that time, the team discusses what must be accomplished to meet the goals of the patient.
Our experience in coordinating and managing the care continuum provides a smooth transition from acute care to skilled nursing facility to home.

We encourage physician involvement in every aspect of care from admission to discharge. At Prescott Nursing and Rehabilitation Community, we believe that active physician involvement improves the clinical outcomes of our patients.
The patient’s physician of choice is welcome to provide services at the Facility. Since many physicians do not provide services in nursing facilities, we have compiled a list to offer patients a choice.

We are committed, at Prescott Nursing and Rehabilitation Community, to being available 24 hours a day and 7 days per week to accommodate your needs. Our knowledgeable Admission Director is always available to talk with you over the phone, give you a tour of the facility, or visit the home or hospital.
Upon admission, we meet with the patient and family member to provide an orientation to the facility. This includes: a resident handbook, information about various payer types and payment requirements, and information about care and services.

Prescott Nursing and Rehabilitation Community is conveniently located Prescott is located just 25 miles from Minneapolis/St. Paul, MN; 16 miles south of Hudson, WI, on Pierce County Hwy “F”; 16 miles west of Ellsworth, WI on US Hwy 10; 35 miles north of Maiden Rock, WI on WI Hwy 35. From Downtown Red Wing, MN take Hwy 63 following the signs to Wisconsin. Cross the bridge over the Mississippi River, continue to WI Hwy 35. Travel west (left) on WI Hwy 35 to Prescott (27 miles). From Hastings, MN, travel north on US Hwy 61 across bridge over the Mississippi to stoplight at top of long hill. Turn right onto US Hwy 10 and continue to bridge over St. Croix National Scenic Riverway and stoplight. Proceed from downtown Prescott to facility.
