Medical Rehabilitation Services

At Crittenden County, we are dedicated to rehabilitating each patient in an environment which promotes independence and safety. Licensed nurses are on-site around the clock.

Our medical rehabilitation program provides high-acuity nursing care for post-acute, clinically complex patients. With a focus on short-term rehabilitation stays, many of our patients return home independently.

Specialized Services

Care Pathways Program for Discharge Planning
Crittenden County is a leader in care coordination. We utilize a program of Care Mapping to develop individualized care plans which specify clinical and rehabilitation goals that are measurable and drive the management of each patient’s care.

Through our Care Mapping process, the discharge disposition of the patient is discussed with the inter-disciplinary team within the first 24 hours of admission.  At that time, the team discusses what must be accomplished to meet the goals of the patient.

Our experience in coordinating and managing the care continuum provides a smooth transition from acute care to skilled nursing facility to home.

The patient’s physician of choice is welcome to provide services at the Facility. Since many physicians do not provide services in nursing facilities, we have compiled a list to offer patients a choice.

Whether you’re considering a skilled nursing facility for a loved one who is unable to safely manage at home, or due to a sudden illness or injury, we are here to help – whenever you need us.

We are committed, at Crittenden County, to being available 24 hours a day and 7 days per week to accommodate your needs. Our knowledgeable Admission Director is always available to talk with you over the phone, give you a tour of the facility, or visit the home or hospital.

Upon admission, we meet with the patient and family member to provide an orientation to the facility. This includes: a resident handbook, information about various payer types and payment requirements, and information about care and services.
